Nova Dental Care makes every effort to simplify the entire dental procedure process for our patients. The following concerns our financial policies, insurance coverage, cancellation policies, forms, and information for first time guests.
First Time Guests
Our first time guests will undergo a thorough oral examination by our general dentist. This examination is conducted to evaluate the health of the teeth, gums, and overall mouth. After the examination, dental procedures can be performed the same day. If the examination, however, finds that a treatment plan should be put into place or a major dental procedure needs to be performed, then a future dental appointment will be scheduled.
Prior to any procedure, please inform our dental staff if you suffer from any medical condition, such as diabetes or heart issues, or if you are taking any type of medication.
If you have recent X-rays that were taken by another dentist or a physician, please feel free to have them forward those to us or bring them with you to your dental visit.
All patients under the age of 18 must be accompanied by a parent or legal guardian.
New Patient Forms
We want to create ease for every appointment you have with us. Filling out these online forms, will help us to start your treatment faster.
Financial Policy and Insurance Coverage
At Nova Dental Care, we accept cash and Visa, MasterCard, and Discover credit cards as methods of payment. Along with these payment options, we also take CareCredit.
Our Houston dental office is in-network and accepts most dental insurance companies and PPO plans. Contact our dental office to find out if your dental insurance is accepted by our clinic. We also accept DentaQuest, Medicaid, MCNA, and CHIP, for patients 20 and under. If there are any changes made to your insurance coverage prior to your dental appointment, please contact us at least 48 hours prior to your visit to inform our staff. This will help reduce your wait time.
Medicaid Patient Forms
For our Medicaid patient, this is a “Main Home Dental Form” printed out and filled out for patients who are not assigned to our office. Parents need to call insurance to make provider changes before coming to our office.
We have a COVID19 questionnaire form we want our patients to fill out and submit to us before their appointment.
